HELP!!!

I don't know what to do anymore and I'm getting worried. For about 3-4 weeks, every time I go poo blood is on the toilet paper when I wipe. This has happened before but only lasted for a couple of days maybe a week. But there would only be a little bit of blood. This time there's a lot and I'm starting to get worried. I don't know what to do and I need some help and advice.
