how to get help when you're injured and can't work?

My SO is a juvenile corrections officer and he injured his shoulder  ( outside of work) so now he's on unpaid leave. He isn't allowed to return to work until he's recovered and according to his orthopedic doctor he'll be needing 3 weeks of physical therapy or surgery if that doesn't work. I have no idea how we're going to get out of this situation we have no money for his treatments so then I worry about him ever going back to work and we have a 6 month old. Basically the only type of help we're getting is foodstamps. Has anyone been in a situation like this before is there some type of medical assistance program we can apply for ?
