Phone Number?

I like this guy. Really really like him. It's more than a crush we have a really good connection but we both have anxiety and are pretty nervous to say anything. Tomorrow is my last day seeing him until next summer, and I really don't want that. So I want to give him my #, the thing is, I want to do it so he sees it when he gets home from where we are. I don't think he knows I like him but I think he likes me, but I know I wouldn't be able to handle it if he said no to my face? How do I give him my phone number in this way? Any suggestions I have so much anxiety
