26 weeks pregnant and bleeding

I am exactly 26 weeks pregnant today and just started bleeding. I had some bleeding earlier in the pregnancy from a subchorionic hemorrhage, but I was told that was resolved and haven't had any bleeding in a few months. What could be causing the bleeding at this stage of a pregnancy?
