OB doctor or midwife

I’m 25 weeks pregnant and I’m not sure if I should switch to a midwife. I have a doctor right now but I can never get in touch with her. When I call the hospital for something I’m transferred to a nurse (which isn’t a big deal) but the nurse would say “i would speak to your doctor about it if your really concerned about it” but I don’t know what to do or if it’s too late to switch to a midwife.
