Transitioning to crib?

Question

My baby is 6 months old, born 5 weeks early, and ever since he was about 2 months he has been sleeping in his rock and play next to me at night so I can get up and feed him etc. he also had severe reflux so sitting up a little was the only way to get him to stay asleep. 
Wondering how other mommas transitioned their babies into cribs? He used to sleep in his crib and he didn’t have a problem and now he only sleeps in it about 3 hours every night IF I can put him down without him waking up. 
Advise would be appreciated!

No advice but right there with you, Mama!🙈we transitioned cold turkey out of the rock n play two weeks ago and out of a full swaddle 3 weeks ago. We have had some good nights and some terrible but it had to be done! Magic Merlin sleepsuit is helping somewhat and our pediatrician told us no for a crib wedge so he is lying flat in his pack n play. Plus we are in the midst of his 4mo sleep regression. Coffee and willpower is the only thing keeping the rock n play away and in the closet😩

My son is 4 months and sleeping in his crib, waking up twice now (I think we’re ending the regression). He also has bad reflux so we bought a crib wedge so he can be a little elevated. He’s not swaddled anymore but we use the Halo sleep sacks and he seems to sleep well. Maybe try the wedge? 
What meds are you using for reflux, if any?
