Breastfeeding. Nipple pain

Anyone else have sharp nipple pain and nipple sensitivity? I am breastfeeding. The pain is random and only lasts a few seconds. Breasts are becoming a bit itchy at times. I can't wear any shirt or bra even slightly tight that presses on the nipples for long because they get even more sensitive and begin to hurt. I feel like I have no bra support because of this so breasts feel like they're sagging. Any recommendations?

I use lanolin creams after feedings. I use nipple shields to breastfeed. I use disposable breast pads to capture leakage and change out often.
