Awkward situation with OBGYN

I volunteer to help with my daughters carpool some mornings. The carpool line cuts through a parking lot between two buildings. My OBGYN apparently has his kid at the other building and I saw him one day because he parked right across from me doing carpool. I didn’t know what to do and just pretended to not see him. He had a mask on and I wasn’t sure at first and didn’t want to stare. Now it keeps happening. He keeps parking where I’m stationed and sitting in his car fir a few minutes before getting out to walk his kid to the other building. I see him almost once a week for the last 2 months and haven’t said hello but I know me. It’s unavoidable that we’ve made eye contact and neither of us acknowledged the other. I feel really awkward now. We are like 10 ft apart and just stare at each other when we catch each other’s glance. To make it more awkward I had a chemical right after the first time I saw him and we spoke on the phone about it, then I saw him right after that but didn’t speak. Now I have a yearly appt and will be face to face. Should I just mention it or ask so it’s not so awkward?
