To be honest, in a relationship your body changes. People put on weight or lose weight, people get wrinklier, they change fashion sense, hair styles/colours, they might get tattoos or piercings etc. Relationships are more than looks and whether you are someone’s “type” and tbf on porn, most women have fake tits and butt implants - it doesn’t necessarily mean he wants that. 
I wouldn’t worry about it because he clearly is attracted to you and your personality and that matters more. Relationships might start with an initial attraction but thinking someone is fit doesn’t sustain a relationship, it’s how you communicate and grow with each other.
I mean let’s be honest, you see elderly couples who have been together since their twenties - what they look like now is different to what they looked like back then and they are still together.
