Depends on what time of day your calling. Just like other business there are peak times where you can expect to have a long hold time. I know at our office if you call with in first hour of opening, it’s not uncommon to hold for 30 mins. When our phones start, I’ve had 18 people on hold before and we have 3 people answering them. Lunch time also tends to be busy. In my experience, afternoons are probably the best time to call if it’s not urgent. If it’s every single time you call, I’d say it’s normal for their practice and they’re probably understaffed. Healthcare is really understaffed right now and long hold times are an unfortunate result of that.

What?! No, I think I've waited maybe 5 mins before someone is back on the phone for me. Or I can send messages via their online portal to my provider.

No, that’s unacceptable and probably due to short staffing on receptionists (or lazy receptionists)
