Milk coming out

I’m Pregnant with baby number 4 I’m 19w 2d. I was placed on pelvic rest for 2 weeks. They did an exam yesterday on me (ending of my pelvic rest) and I’m 1/2 centimeters dilated. I’ve never had my kids without induction and my body has never gotten to 5 centimeters without a fulybulb so my OB isn’t worried about the dilation. This morning I realized I leaked milk last night my shirt has dry spots at my nipples and my nipples are crusty white….should I go back to my OB with this concern or just write it off?
