How do I leave him?

We just moved closer to my parents. He has no family close by, he isn’t close with any of his family. We’ve been having issues since we’ve been together and I accidentally got pregnant after a very short time of meeting him. I tried everything. I tried communicating, changing, everything but nothing is working. We have a six month old and we are barely making ends meet. Our bank is together and his name is listed as a roommate on the lease

I know I don’t want to be with him. How to I leave him?
