How would you word an email asking about maternity leave?

I work from home. I reached out to someone abt maternity leave and they told me to email etc etc and cc etc etc. I have never asked for maternity leave so not sure how do I word it? I am part time now but I did work full time for half a year. Can anyone give me an idea how to professionally word it? I want to ask if they pay, how it works for part time, how long it is etc.
