Husband makes me so sad

When we fight, he tells me I’m crazy and I have split personality. No matter what caused the fight, big or small, he always gives a low blow and makes his own diagnosis of me. I do admit I have untreated high anxiety. But I think saying I’m crazy and need to be in a mental institution is a stretch. Anyway, Today I called and got a doctor to take me as a new patient. I didn’t have one before. When he came home from work today I told him I had an appointment but since I am new, I can’t be seen until May. Then during a fight tonight he said “I’m so sick of your twisted brain. May can’t come soon enough. You need to change”. In front of my step son whose 8 and while he was holding our 1 year old daughter. He told me I mentally abuse him because I make comments “as often as I can”. Anyone else has this too? How can we resolve this? He refuses therapy.
