What is this!?

Anyone have any ideas what this rash could be? My husband has had this for months now. Spots that itch like crazy, break open and bleed, and are mostly on the ankles, under his socks. We've ruled out just about every bug out there and various other common skin conditions. It doesn't spread at all but won't go away either. He hates the doctor and won't go so I'm hoping someone has experienced this before so I can get some answers for his stubborn ass. 😅
