Needs some advice on my 4yr olds hearing

My daughter started school this year in August and got her first “known” ear infection. She has been sick off and on, as expected with young kids. She hasnt been sick in about a month but we have struggled with her hearing. She constantly is saying “what? I cant hear you” We have to yell a lot for her to hear us, even her teachers have said they have a hard time getting her attention. She isnt being defiant either.

After 3 weeks of this we finally took her to the Dr and they did test on her ears which passed. They didnt see anything really wrong but said she had some clear fluid in her ear. She was recommended childrens Zrytec for 3weeks to dry it up.

Im just wondering can clear fluid cause hearing problems like this? or should we get a second opinion?
