Diaper rash?

I’ve been giving my son (8 months) baby oatmeal every morning for maybe a week now and I noticed the past couple days he’s been pooping way more often. He’s breastfed and normally would poop 1-2 times a day (and would sometimes even skip a day) but it’s been like 4-5 times today so far. His butt has gotten so red over the course of today and has gotten so tender. His poop was also sort of green a couple times. He grunts and screams when I wipe him, this last diaper change I just rinsed his butt off in the bath instead and I feel so bad. I’ve been putting diaper rash cream on but it doesn’t seem to be helping yet. Do you think it could be the formula in the baby oatmeal? I would pump and use my milk for it but I don’t respond much to the pump and my supply has been going down, which was my reason for wanting to give him a bit of extra nutrition via baby oatmeal with formula because he refuses a bottle. He eats other stuff throughout the day, I’ve just noticed he’s been slimming out the past month or two and figured this would be a great idea. :(

Does it sound formula related or could he be sick?

Is there something else I should be doing in addition to the cream to treat it?
