Can i request domestic violence records?

If anyone can help, a couple years ago I was the victim in a domestic violence incident with an ex. He was arrested. I want to request the records of the incident and his arrest which is allowed in my state, but it says you have to get it notarized, and it says “the subject of the record must sign a release form in front of a notary public” so does this mean that he would have to get it notarized because he was the one arrested, or would I also be considered a subject of the record and be able to request it because I was listed as the victim? Thank you
