Louisiana mama!

If I have an established residence in another city while the father is in another (Shreveport, Lafayette) but we break up, can I switch doctors from the one where he's at and have my baby where I am and keep her here with me? I think we're about to break up and I'm scared I won't be able to be home and have her with me. He will be on the birth certificate but she won't have his last name
