Custody documents

Seeking advice about custody related issues. When a custody order is put into place and a modification of custody is made does that make the old custody papers go away??

Exaple...when my husband gained custody of his 3 children it was 50/50 with him having placement. She was to have every other weekend visits....call the kids 4 times a week and access to all medical. Well ab 3 yrs ago we had to go back to court bc she was abusing the kids. So now the modification says....supervised visits only with my husband supervising and counsling for her. Nothing else. Do we still obey the first order too as well as the few modifications??
