Notification will not go away?

The glow app keeps giving me a notification to "Schedule you annual well woman exam" and this won't stop. I've even already met with my doctor. I click it and turn it off, rescheduled it for another day, and it still pops up and it's listed as off on the settings. I can't turn it on there (as its set for November 3rd now) and can't change it because the title is too long but i can't change the title and there is no delete button.

Also sorry, i didn't see a technical/app help section.
