Who is mostly responsible?

Scenario:
A manager at a warehouse orders some machinery from a large company that’s known for being a hit or miss as far as quality but the prices are unbeatable. Although the manager wasn’t told NOT to order through that company they know that there’s a chance of receiving low quality &/or faulty machinery. However, the budget is tight so they figured the machine will do until they can afford better. 
This person is: Person A

A mechanic at the company that produces the machinery was having a lonnnggg day. After checking for malfunctions in machines for hours he decides that skipping the last one so he wouldn’t get off 30 minutes late wont hurt. He reasons that because all the machines were made the same way, if 9 out of 10 worked then the one he skipped will probably work as well. Furthermore, he has no way of knowing when the machines will be bought and sold and assumes that he can come back tomorrow and do it, unfortunately the machine he skipped gets sold and shipped off that day.

A worker at the warehouse that bought the machine notices that the new machine they just got isn’t quite working correctly. While company policy is to report any malfunctions, they have no way of knowing the extent to which it was broke so that worker simply decides to get a different machine. They complete their job, move on with their day and DON’T report it to their manager for further investigation. 
This person is: Person C

A few hours after Person C, another warehouse worker comes along & uses the same machine that was previously mentioned. Unfortunately the machine malfunctions which results in the loss of this persons legs. They will now be paralyzed from the waist down for the rest of their life.

Who (if anybody) is mostly responsible for Person D becoming paralyzed?

Person C. Although persons A and B were irresponsible, person C is the only one who knew for a fact that something was wrong with the machine and didn't report it.

B and C in my opinion. A was just being cheap. B wasn't doing his job, C was being negligent and also not doing his job.

I'd say B, first and foremost.  They would be held accountable for this as they fudged the reports saying this machine was working.  C would also be held partially responsible for not reporting the malfunctions when noticed.

There was no option to pick all of them. Person A is an asshole for ordering from a company who is not reliable bc of irresponsible people like person B who don't check all of the equipment. Person C has a more direct responsibility due to his lack of reporting a malfunctioning piece of equipment bc that's basic protocol in any business bc other employees could be harmed.

It's not the individuals' faults. It is the company that makes the crap equipment's faulty as a legal entity. Person A is not at fault because even if they buy something that they know isn't the best, it should still be safe since it has a warranty. It just won't last as long and will need more repairs. Person B made a bone head move that was selfish, but it's very unlikely that that us exactly what caused the problem. It would take a whole team much longer than 30 minutes to quality control check a machine that large, it's likely a design fault. Or if it is the kind of machine that only takes 30 minutes to qc, it's not the kind of thing that every machine gets checked. Person c didn't even know what was wrong with it. They just knew it wasn't doing it's job. They didn't know that safety devices were disabled. The last person also needs to check that stuff and didn't apparently.

Person B and C. Although Person A did take a gamble with quality, the others should have properly completed their jobs and reported any missteps. Person B should have at least told someone he wasn't going to check that machine due to fatigue which resulted in a lack of competency. Person C should have definitely reported that machine or at the very least put a simple "out of order" sign on it. Person D was just trying to do their job and experienced a tragedy. I would say Person B was potentially the most at fault, but I am on the fence with that one.

Did this really happen to someone?! That's a terrible coincidence of irresponsible people. I feel like the biggest part of the fault would rest with the factory inspector because it was actually his job to make sure the machine functioned correctly.
