Who is mostly responsible?

Scenario:

A manager at a warehouse orders some machinery from a large company that’s known for being a hit or miss as far as quality but the prices are unbeatable. Although the manager wasn’t told NOT to order through that company they know that there’s a chance of receiving low quality &/or faulty machinery. However, the budget is tight so they figured the machine will do until they can afford better.

This person is: Person A

A mechanic at the company that produces the machinery was having a lonnnggg day. After checking for malfunctions in machines for hours he decides that skipping the last one so he wouldn’t get off 30 minutes late wont hurt. He reasons that because all the machines were made the same way, if 9 out of 10 worked then the one he skipped will probably work as well. Furthermore, he has no way of knowing when the machines will be bought and sold and assumes that he can come back tomorrow and do it, unfortunately the machine he skipped gets sold and shipped off that day.

This person is: Person B

A worker at the warehouse that bought the machine notices that the new machine they just got isn’t quite working correctly. While company policy is to report any malfunctions, they have no way of knowing the extent to which it was broke so that worker simply decides to get a different machine. They complete their job, move on with their day and DON’T report it to their manager for further investigation.

This person is: Person C

A few hours after Person C, another warehouse worker comes along & uses the same machine that was previously mentioned. Unfortunately the machine malfunctions which results in the loss of this persons legs. They will now be paralyzed from the waist down for the rest of their life.
