What do you do?

What do you do when your supposed to be boyfriend is calling you a friend to family members and his friends, but calls you his woman when you're together??? I feel some type of way because it doesn't really seem like a relationship now, he constantly refers me as a friend to people he talk to, haven't seen him since July of last year. He had cancer on his kidney, so I wasn't allowed around him, now he has cancer on his stomach, probably won't be able to see him then either. So if I'm just a friend to him, does that explain why we don't spend as much time together??? Been with this guy for 6years as a long distance relationship and yes we've seen each other in person, within the 6 years.
