Have you guys gave your 4 month old cereal? If so explain how below.

Did you use bigger nipples, put it in the bottle, spoon feed it?

Way too early, wait another 2 months. If baby is waking in the middle of the night  it's because they're going through a growth spurt and need the nutrients. Cereal doesn't give them that nutrients.

I mixed it up for my son with his formula in a bowel and spoon fed it to him or mixed it with his purée when he was 5 months that’s when we started even though I got the ok at 4 I tried it then with him and felt he wasn’t ready yet so waited another month

We did both. When we put it in the bottle we used size 2 nipples.

We did oatmeal instead of rice cereal as recommended by our doctor as well as  barley. 
My son had really bad colic issues , we did it closer to 5 months and only like a few teaspoons in his formula not enough to make it thicker, it was a way to help his colics and acid reflux. By 6 months or so, i did start making it thicker and I switched to a Y nipple from Dr Browns. Since he was eating, he didnt care much for his formula so I would add some of the oatmeal and veggies/fruit to 4oz  formula and he ate that 3-4 times a day plus baby food. It was  a thinned out yogurt consistency. He enjoyed the different fruit & vegetable flavors. Once he started eating he did not want formula at all, adding fruit and veggies to his formula and oatmeal got him to eat his formula.

I may end up doing it with my daughter if she's still having issues gaining weight and her doctor recommends it. I would only do spoon fed. If it can be avoided, though, they recommend waiting til 6 months before introducing any foods so the baby's digestive system is better developed for it
