Dcumentation for work?

I need to type up some documentation for my manager to sign saying I’m a student and won’t be able to answer my phone while I’m at school.   Because if I’m in class and I don’t answer my phone they might fire me..?
I’d like some input on what to say.

Go to the office at school and get a VOE (verification of enrollment) to give to him. If he needs additional documentation then attach the cell phone policy from the schools handbook

That's something that you'd get from the school, not something you'd type up yourself. Your school would know what to say.
That being said, if you explain to your professors that your job requires you to answer occasional phone calls, 99% of them are fine with you stepping out to take a call. Most professors don't really care what you do, as long as you aren't disrupting the class.
