Mother dies from mystery opiod overdose - Thoughts?

Question

Recently in my city a woman in labor was administered fentanyl as well as other medications that are unknown to the family. She was given fentanyl in the beginning of her labor as well as again as she progressed. 
Eventually she passed out, had a seizure, and unfortunately passed way.

A traumatized family trying to find someone to blame. They absolutely did not give her 100 millimeters of Fentanyl. They gave her 100 MICROgrams. That is not enough to cause an overdose. It is a relatively small dose, and would not be enough to kill her. Chances are she had another complication related to birth. If it was something as simple as an overdose, they would have given her Narcan and she wouldn’t have died 🤷🏼‍♀️.  You decline before going into cardiac arrest with opiates. Many places allow family to remain in the room, and the family would have been free to leave if they didn’t wish to watch her be coded. It’s proven to give family closure to see everything possible was done.  They also don’t have time to focus on shooing people out of the room. Their focus is on life saving measures, which don’t include cleaning her up...
I feel bad for them, but the fact that they are claiming she was given 100 ml of Fentanyl shows how uninformed they are.

She probably did have an allergic reaction or complications from the IV. The flow rate could have been incorrect. Some medications will still hurt when being injected.
Now if a crash team comes to do CPR they aren't going to care about the family members their main focus is on the patient. I would only assume it's basic knowledge to not get in the way of the nurses. I would have left the room if she was having a seizure. It's really unfortunate):

I think its important to keep in mind that this is the families version of events. Presumably not medical professionals, and they dont know that the 2nd medication was also fentanyl for sure. An OD in a hospital is pretty unlikely given they could have administered narcan and revived her if that was the case.
 However, the Drs history is sketchy as hell and he should not be practicing.
I really do feel for the family and hope they get legitimate answers.

I cant comment on the medication reaction cause I'm not a doctor and dont know the proper doses for that medication but why THE FUCK was the doctor still practicing with all those charged?! SEXUAL MISCONDUCT? And hes working as an obgyn?! That's shocking and malpractice on the medical board and hospitals part.

Maybe she had some sort of underlying condition no one knew about? People with an underlying/ undiagnosed condition are basically ticking time bombs. Or she had some kind of reaction?
If the doctor is undergoing a court case why is he still practicing? The nurses would just give things they're not comfortable with, they'd approach the matron of the ward if they were in doubt.
I can't help but wonder why the family hung around to watch this instead of leaving the rooms themselves. Perhaps the medical staff were too busy trying to save that her life than to instruct what should be obvious.
They definitely should have cleaned her up though without having to be asked, for the families to say goodbyes.
Without actually being there/ hearing the other side this is just one sided with no medical facts as to what happened. I really don't mean to sound insensitive but the story might be exaggerated to a degree as well, as they said the deceased "screamed in pain" whilst just getting an injection?
It's a very sad story regardless.

She didn't die from overdose. She had another complication. She would have had signs of overdose and would have been administered Narcan if it was an overdose.

No. 100ml is a huge volume. A nurse could not even get that much into a standard syringe. 30ml is 1 ounce. So 100 ml is over 3 ounces of liquid. To put that into perspective, 1/2 cup is equal to about 120 ml. That’s absurd. Likely they meant micrograms, which is a normal amount of medication and it would be a in very tiny amount of liquid, like a single ml or something. Something went wrong here, but I don’t think it was the fentanyl the nurse administered. I’m sure there will be an investigation into what happened.

I’m not saying what happened to her, that’s up to the autopsy and what the hospital did wrong will be up to whoever investigates.
FYI, I never heard of cleaning up bodies before autopsy; we never did. If a family wanted to see their loved one, we’d inform them of the condition of the body and why it wasn’t cleaned. 
Regardless, I hope family get answers. Maternal mortality in the states is so high, and so many families have suffered through it.

100 ml of Fentanyl 🤔 I highly doubt that. Try 100 mcg. No one would ever give 100 ml. 
She didn’t die of an overdose. The family is very misinformed about medications and dosages apparently. 
They are grieving and looking to blame someone for what happened.

This story brings up more questions than answers. 
Main ones being: 
Dosage
Time of dosages
Time between the end of CPR to time of C-section
And the biggest one: Where the fuck was the on call OB? 
Regardless of those questions. The drug she was given is only ever given (to my knowledge) in highly monitored situations, and not 100ml of it. Maybe 5ml at most with 6-8 hours between dosage (depending on the condition) 
In it’s pure form a grain the size of salt can kill a fully grown human. So there are a lot of questions. 
Hopefully the investigation brings to light all the wrong doing because at this moment, there are too many unanswered questions.
