That looks like an allergic reaction to me. Very common to be allergic to penicillin. I personally was as a baby.

Make sure he gets probiotics. Antibiotics can mess up the gut very badly. Yogurt is good for this as well as probiotic drops or powder. Hope it gets better!

Yup both my kids do to 2 different families too. Unfortunately doctors think most moms fake it so the push penicillin. My daughter suffered twice before we switched doctors

If its only in his diaper area, definitely not an allergy. Antibiotics cause very acidic diarrhea and can burn the skin. This is an adverse effect of the medication but not a rash. Change diaper ASAP after bowel movement. Apply zinc based diaper cream with each diaper change and do warm water soaks with baking soda twice daily.
