Bf not caring for his health!! Help!!

So my boyfriend got cancer while in prison, and received free chemotherapy while there as well.

He survived the cancer almost 3 years ago, but has a port in his chest that was to target radiation with a tube connecting under his skin. He also was left with a heart condition after the cancer that can cause irregular heartbeats.

After his chemo, he was supposed to get the chemotherapy port removed from his chest after he left prison, ALMOST 3 YEARS AGO

I repeat, he has needed SURGERY FOR ALMOST 3 YEARS

continuing to avoid the surgery can cause him to pass in his sleep, and that risks increases because of his heart condition. Also it is bright red and painful now, so we are thinking it got infected under his skin. He is very prone to blod clots now as well

He doesn't have any insurance and has been denied for Medicaid. When he had Medicaid previously it only covered some things but he still had missed appointments.

My family and I have referred him to pretty much every free/low income clinic and he WILL NOT CALL THEM!! he is avoiding any medical help and told me he isn't afraid to die!!

Every night I am worried he will die and if I can't hear him breathe while he's sleeping I need to wake him up . It's causing me so much stress. We have a 5 month old baby together!!

I have been begging him to get medical help and he always says he will call the doctor and never does. I understand he might be worried about getting bad news or spending money but I'm scared he will die I he doesn't get help!!!!

I just don't know what else to do or how else to help him!!!
