Is it offensive to ask are you on a diet? Like if you offer a food to someone and they decline to accept the food and you ask are you on a diet?

kinda, mostly bc it's unnecessarily nosey. if someone says no to food, the person offering should just take it graciously rather than pry.

Someone may decline food for allergies, religious beliefs, etc which is not the persons business that’s offering food

In the scenario given, yes it is rude. They don't owe you an explanation for declining food that you offer.
Now, when planning to have guests over and will be serving them food, finding out if they have any dietary restrictions is a nice gesture. But them saying they can't have dairy isn't the same as asking them if they're *dieting*.

Why would you ask if they’re on a diet? Why not “Are you hungry?” or “Do you like xyz food?” or “Do you have dietary restrictions?” or “Are you allergic?” Jumping to diet is unnecessary. I personally expect someone to ask if I’m even hungry before they even offer me food.
