In short she has Alzheimer’s, and her husband not only has girlfriend but moved the girlfriend into the house. Is this cheating? 
What happened “for better and for worse until death do us part.”

Alzheimer’s runs in women in my family. It hits early and it hits hard. 
I’ve told my husband, if it happens to me and and it gets bad, leave me and be happy. Don’t visit when I forget who you are, don’t force yourself to suffer through watching that out of loyalty, don’t feel obligated. Love me, but leave me. 
I’d be fine if he found someone else to be happy with.

Idk if I would call it cheating because we don’t know what arrangements they made before her conditioned worsens. 
I will say this though. I think it’s EXTREMELY disrespectful to move his gf in her home. He could’ve gotten her a condo or whatever. You don’t move your mistress in mayo your sick wife’s home. 
I also want to know if he’s setting up businesses and such with his personal money or with the gfs money. Because if its with the B/B and his money. That’s disrespectful too

His wife doesn't know she is his wife and he hasn't abandoned her. He lives with her and cares for her and his gf helps care for her as well.  If I didn't know who my partner was, or know who I was and he was caring for me I would be fine with him finding someone to support him in that. I can't imagine how awful it must be to see your wife slip away and I don't judge him for it at all.

I don’t think this answer is so black and white. We don’t know what kind of arrangements they had discussed prior to her Alzheimer’s getting worse.  I get how some could see this as cheating, but being a caregiver for someone is very hard in a person, especially if the caregiver is a spouse.  I can see why he’d reach out and become close with someone else.

If they had a prior arrangement then whatever. If not...it makes me uneasy. It’s not something I would do if my husband became mentally incapacitated. I understand the person isn’t mentally there anymore, and I understand needing companionship and love, but I wouldn’t even be able to fall in love with someone else while he is still alive. I married him because I want to take care of him if he’s in a vulnerable place. Of course we all hope that never happens. 
On a side note, this is a reminder to everyone to make a living will! Do it while you’re young and healthy. And have hard conversations with your loved ones about your wishes if you ever aren’t able to make decisions for yourself.

I think I can’t begin to understand how hard it is to watch your spouse deteriorate like that and I feel for him and his family. I don’t think I have a one size fits all answer other than that.
