Why does she do this

My daughter will sleep fine when she’s in my arms or on my chest and as soon as I put her in her bassinet she wakes up 5 minutes later how do I break this habit

How old is your baby? This is totally normal. Look up the ‘Fourth Trimester’. Baby feels safest when she is close to you. This isn’t a ‘habit’ you need to break, just give it time. 
If you have a sling/stretchy wrap, babywearing can work wonders for this stage. 
Our daughter was about 2.5-3months before she became comfortable with sleeping for extended periods in her own bed.

She senses she is in a different environment/position and is woken. If you can have her fall asleep lying down already that would be ideal. I would suggest the pick up/put down method(google will give you specifics) as a gentle sleep training for your LO. The ultimate goal would be to have the baby fall asleep on it's own so to try it now is a good idea. My baby did the same for a long time and instead of sleep training I just gave in and held her and that created a whole other set of problems so try your best to not fall into that all the time. Of course theres times you just need a rest and will hold her but the majority of the time at least try to put her down prior to her completely falling asleep.

My daughter used to do that, I put a fleece blanket on the matress so it wasnt too cold when I out her down and she stays asleep all night. Worth a try

If she wakes up when you set her down, keep your hand on her belly and let her know you are still there. When she drifts off keep it there for a min or 2. It took a few times with my daughter but she finally started sleeping in her crib
