What should I do?

I have a 2 and 3 year old and have 50/50 Custody with mom. We swap every Friday. Every time I get them back from there mothers house my daughter has a horrible rash so I take her to the doctor every time I have her to have it Documented by a doctor recently she said she took her to the doctor and the doctor gave her rash cream. She had this “rash” for almost 4 weeks. When I got her back that next Friday and the rash wasn’t getting any better I took her back to the doctor, the doctor said that she never took her and to be seen and that she had a yeast infection that turned into a Bacterial Infection. Turns out she gave me Back the rash cream that I have given her the time before to use. She says she gets the rashes because she always has Diarrhea (she never does when I have her) she Just recently found out she was pregnant again (with someone else) she lost her house because she wasn’t paying the rent, in the last two years she’s had at least 6 jobs and she is getting ready to lose her car. Who she has been staying with a few days a week (she said they can only stay for a few days at a time so she couch hops) but the lady has reached out and said that she does not bath them feed them and all she does is yell at them and make them sit quietly at home or put them in a bedroom. She’s currently staying in a one bedroom with her boyfriend her and my two kids on one queen size bed. And at her mothers they sleep on the couch/floor. She goes back-and-forth between this lady’s house and her mothers house but they both say they can only deal with her for a few days she’s too much to handle But neither wants to see the kids have nowhere to go when she has them. They said they have talk to her about me taking full custody and she says she won’t let it happen because she wants my child support (And if she doesn’t have the kids she knows she’ll be homeless) what should I do? Should I call cps? My lawyer said the next steps are to file for emergency custody which I am fine with doing but it could take a week to two weeks.

Also to add I own my home and the kids have there own room And I can financially take care of them full-time.
