Please help I need advice

I know i’m gonna get some judgement for what i’m about to share and i understand, but i have a 5 year old daughter and i haven’t taken her to get her vaccines since she was one due to me being vaccine hesitant and over time i’ve gotten more understanding on why i NEED to vaccinate her so i am willing to give her all her vaccines now but the problem is i’ve read that cps can be called on me and have my child taken away? and im just not sure how im supposed to even go about this, do i just call her pediatrician that i used to take her and make an appointment or how am i supposed to explain over the phone that i didn’t vaccinate my child and for so long and now i need to?? please I want her vaccinated im just not sure how to go about this !!
