Who gets your kids if you and their father dies?

Question

Do you have a will or legal document indicating who would get guardianship of your kid(s) if something happened to you and their father? 
If yes, who is it and how did you decide?

I'm not trying to be morbid here. It's a decision we're trying to make right now, and I'd like to hear others' stories.

Hubby & I am hoping that my sister-in-law will take them but we haven't seriously made plans.

My son's godfather and his wife. We asked them a couple months ago. Not sure what they'll say when we have a 3rd.

my best friend and her husband would get custody of my daughter.

We haven't fully decided just yet. Probably to my mother if she was in a good place or my SOs siblings. They all have children and know my boyfriend enough to raise our child properly.

My mother would get custody of our girl's and if she was in a position that she could not, they would go to their Godmother.

My sister would take custody of our daughter because my nephew is 10 days older then our daughter and they're super close. If anything were to happen to her and her husband it would be the other way around and we would take custody of him.

my boyfriend and I are on his sister and husband's will to have custody of their daughter if anything happens. we are also having a baby in November and we will make arrangements then.

I am 16 so I probably have no say in this(stating that I have never had sex never even kiss a guy on the lips) but if my mom and dad have a legal thing to where I go to my older brother and I could choose where I lived or my brother chooses where I lived if I was to young to choose

My son has several people who would be able to take him. DH's battle buddy in WV and his wife, several of our friends here etc. I personally wouldn't trust my parent's or DH's to raise our children.
