How early can doc see pregnancy on u/s?

My doctor is doing an ultrasound before he places an iud since I haven’t gotten my PP, how soon can they see the start of a pregnancy via ultrasound? What would happen if they missed something super super early and placed the iud anyways?

Sac might be seen even on 3. It doesn't look like baby, but they see that there is something there.
