Smelly baby neck

How does one clean the neck folds on a 3 month old? It’s hard tilting her head back, because she resists and screams. There’s always fluff and nasties there, and I’m always scared she’ll get some bacterial infection there. But it’s so difficult to clean both in and out of the bath. Whenever I’ve tried to clean, I’ve used a cotton pad with coconut oil whenever I’ve tried, as coconut oil is anti bacterial/anti fungal, but I’m successful 10% of the time.

Side of the neck is just about achievable, and below the chin is basically impossible.
