Is it wrong to lie in this case? (TMI-poop)

I have a bit of a predicament. I’ve been experiencing super weird hunger and digestive issues lately. Basically I have had absolutely zero appetite for about 3 weeks, I get dizzy around 11 pm, and finally try to eat something. I do eat super small snacks about 2 times a day, but I’m not feeling hunger cues at all. And almost every single time I poop, it’s diarrhea. I’m talking several times a day. No other symptoms-no fever, stomach pain, cough, etc. I try to make an appointment with my doctor, it has me fill out a symptoms form. One question is if I’ve had diarrhea in the past 14 days. I check yes, and then it says I cannot schedule the appointment. But it’s a Telehealth appointment. Is it wrong to lie about symptoms when I wont actually be face to face with anyone? Obviously I’ll tell the truth in the actual appointment.

Bonus questions: Has anyone experienced this? Any tips on the appointment? I have a history with disordered eating so I’m very nervous about this appointment.
