What does sexually active mean???

Lol I’m sorry I know this is a stupid question... but when a gynecologist/doctor ask you if you are sexually active.. What does being “sexually active” SPECIFICALLY mean ..?

Does it just mean intercourse/penetration & like genital to genital

Or is it any thing that can be(or is honestly..) labeled as sex. So also being oral sex (giving/receiving) or some kind of touching of the genitals.. ( I’d presume only skin to skin contact would qualify in this case..) being of oral, hands , etc...
