Need opinions.

My husband and I usually don't go far enough during sex to make him cum. Usually the condom doesnt make him feel as much as I do. So I wanted him to keep going this time and when we finally did it felt amazing and we were both happy. Problem was, is that when I got off him the condom was gone. We were both in shock looking around. I saw nothing but when he walked out to get our phones I could hardly feel anything except the harder part of the condom all the way up in me. I had to get it out with my finger and I was so traumatized. When I got the condom out, there was cum and a bit of blood... (we even used lube cause I've bled once before and I have pain during sex usually). I'm on birth control but in the week before I had taken it not on time maybe twice (a few hours off what I usually take it at). I want to know if anything like this has happened to anyone, how they coped, and what should I do. This happened Feb 18 2019.
