Bad milk blister won't go away

My nipples were pierced before my pregnancy so I knew that there would be some issues as far as scar tissue was concerned but one of the scars has formed itself into a multi-faceted milk blister. It healed for a little while but seing as it's still scared it became another one. I ended up having to buy a larger flange for my pump because its swollen up on the side of my nipple so when I try to avoid it touching the wall of the flange the other side hits it how on earth do I get rid of this thing? I've tried doing to Epson salt soak but it doesn't seem to work
