MAT B1 Form?

Hey 😊 just wondering where I can get the MAT B1 form from? Just had my 20 week scan. Thought they would just give me one but they didn't and I forgot to ask!

Your GP can give you it aswell. My midwife never bothered to give me one.

I got mine at 25 weeks first baby and 28  weeks for this pregnancy.

Your next appointment midwife will give it to you

I had mine from my midwife at my appointment like 2 weeks after 20 weeks
